Tickets may also be purchased by mailing a <b><u>self-addressed & stamped envelope</u></b> along with the form at the bottom of this sheet to our treasurer, Carol Fullilove, 418 Kenshire Court, Shreveport, LA 71115. Carol works at Arthur Circle Elementary, and will have tickets at school if you would rather come by. If you are mailing the form within a few days before the Thursday, February 16th deadline, you may call in the reservation to Carol at 318-797-0148 or Diana at 318- 573-0388 to prevent the additional charge for late ticket purchases. All reservations must be paid in full, even if all the tickets are not used.</p> <p><u>Checks must be payable to Caddo District PTA and have the qualified signatures on them.</u></p> <p>Tickets are available for $20.00 each. Please purchase a ticket for each of your award recipients and others who will be attending.</p> <p>We encourage the members from each of our PTA units to come early and enjoy visiting with all the other volunteers attending.</p> <p>The Caddo District Board of Directors would like to remind you of standing rules that apply to purchasing tickets:<br> Caddo District PTA Standing Rules, #16. e. and f.</p> <dd>e. Any tickets purchased or reserved before the published deadline will be charged the published price per ticket.</dd> <dd>f. Any tickets purchased or reserved after the published deadline will be charged the published price per ticket plus a $10.00 late fee per ticket.</dd> <a href="Forms/FoundersDay2012.pdf" target="_blank">Founder's Day Ticket Order Form</a> <hr> <h1>Founders Day Recognitions</h1> <p>It s not too early to start thinking about which of your PTA volunteers to honor at the Founders Day Banquet in February.</p> <p><u>There are two ways to honor volunteers and support Caddo District Scholarships</u>: Order forms for both of these are available below the description of each award.</p> <p><b>1. Caddo District Member Scroll:</b><br> For a $10 donation, your volunteer will receive a pin and certificate. This award is available from Mrs. Kathleen Barberousse at Fairfield Elementary Magnet.<br> <a href="Forms/LifeService.pdf" target="_blank">Memberscroll Award Order Form</a></p> <p><b>2. Caddo District Honorary Life Service Award:<b><br> For a $30 donation, your volunteer receives the highest honor awarded by our district with a custom made pin and certificate. This award is available from Eppie Adams, who chair s the life service committee.<br> <a href="Forms/MemberscrollAwards.pdf" target="_blank">Caddo District Honorary Life Service Award Order Form</a></p> <p><b>Louisiana PTA Life Service Award:</b><br> This is the award that can be given by a local unit. It is purchased from the State PTA office at a cost of $40, and includes a pin and certificate. This is a national award that has to come from the National PTA.
